What is the SSS Sickness Notification Form?

The SSS Sickness Notification Form (officially SSS Form CLD-9N) is the document used to notify the Social Security System that you are confined due to illness or injury and wish to claim sickness benefits. Without filing this form correctly and on time, your sickness benefit claim may be delayed or denied.

The form has three parts:

  • Part I — Confined Member’s Notification (filled by the sick employee)
  • Part II — Medical Certificate (filled by your attending physician)
  • Part III — Employer’s Report (filled by your employer)

Who Needs to File the SSS Sickness Notification Form?

  • Employed members — notify your employer within 5 calendar days of confinement
  • Self-employed members — submit directly to the nearest SSS branch
  • Voluntary members — submit directly to the nearest SSS branch
  • OFWs — file through the My.SSS portal or nearest SSS office

SSS Sickness Benefit — What You Can Receive

When your sickness notification is approved, you are entitled to a daily cash allowance equal to 90% of your average daily salary credit for each day of confinement. Key limits to know:

Exception: For hospital confinement, the 5-day notification deadline does not apply. However, it is still best to notify as early as possible.

Note: If the sickness or injury is sustained while at work or within the employer’s premises, the employee is automatically considered to have notified the employer.

How to Fill Out the SSS Sickness Notification Form

Part I — Confined Member’s Notification (You fill this out)

  1. Enter your full name and SSS number
  2. Indicate the date your confinement started
  3. Describe the nature of your sickness or injury
  4. Sign the certification section — you are waiving in favor of SSS all medical information from your physician needed to process your claim
  5. Fill out in one (1) copy
  6. Always indicate “N/A” for sections that do not apply — never leave blanks
  7. Affix your initials on all alterations or erasures

Part II — Medical Certificate (Your attending physician fills this out)

  1. Your doctor fills in the clinical summary — symptoms, physical findings, laboratory results
  2. Doctor checks the appropriate box — Initial, Intermediate, or Final Sickness Notification
  3. Doctor estimates the number of days of confinement and expected date fit to resume work
  4. For prolonged confinement, attach X-ray plates, lab reports, and special diagnostic results
  5. Doctor signs with their PRC registration number

Part III — Employer’s Report (Your employer fills this out)

  1. Employer records the date they received your sickness notification
  2. Employer certifies whether they can or cannot verify the sickness
  3. Employer signs and submits to SSS through the My.SSS portal

Required Documents for SSS Sickness Benefit Claim

Together with the Sickness Notification Form, prepare these documents:

  • Duly accomplished Sickness Benefit Application (SBA) form
  • SSS ID, UMID card, or any valid primary ID (Driver’s License, Passport, Voters ID, NBI Clearance, Postal ID)
  • Medical certificate from attending physician
  • Supporting medical documents — laboratory results, X-ray, ECG, other diagnostic results
  • For prolonged confinement — progress report and additional diagnostic results

How to Submit the SSS Sickness Notification Form

For Employed Members

  1. Notify your employer within 5 days of confinement
  2. Submit the completed form (Parts I and II) to your employer
  3. Your employer submits to SSS online through the My.SSS Employer portal within 5 days

For Self-Employed and Voluntary Members

  1. Complete Parts I and II of the form with your physician
  2. Submit directly to the nearest SSS branch or through My.SSS portal
  3. Bring all required supporting medical documents

  • Processing typically takes 10 to 30 working days
  • You can track your claim status through the My.SSS portal
