What is the SSS E1 Form?
The SSS E1 form — officially called the Personal Record Form — is the registration document used to apply for an SS Number in the Philippines. Every first-time SSS member must complete the E1 form to get their SSS number, which is a permanent lifetime identifier required for employment, accessing benefits, and applying for loans.
You need the SSS E1 form if you are:
- A new employee registering with SSS for the first time
- Self-employed and applying for SSS membership
- An Overseas Filipino Worker (OFW) registering for SSS coverage
- A non-working spouse applying for voluntary SSS membership
- Any Filipino citizen applying for an SS number for the first time

Who Needs to Fill Out the SSS E1 Form?

Important: New registrants over 60 years old who are not surviving spouse pensioners or guardians of pensioners are not qualified to apply for SSS membership.
How to Fill Out the SSS E1 Form
- Print the form in two (2) copies — you need to submit two copies
- Use black ink only and print all information in capital letters
- Fill in Part A — Personal Data: full name, date of birth, sex, civil status, nationality, religion, TIN (if any), place of birth, home address, contact details
- Fill in Part B — Dependents/Beneficiaries: list your children and other beneficiaries with their dates of birth and relationship to you. Use an additional sheet if necessary — always write “N/A” for sections that don’t apply
- Fill in Part C — Self-Employed/OFW/Non-Working Spouse section if applicable — include your monthly earnings and Flexi-Fund program details
- Fill in Part D — Certification: sign the form to certify all information is true and correct. If you cannot sign, affix your fingerprints in the presence of SSS personnel
- Attach all required supporting documents (see below)

Submit any one (1) primary document from the list below:

If none of the above are available, submit any two (2) supporting documents both showing your correct name and at least one showing your date of birth — such as a School ID, Senior Citizen Card, Police Clearance, or Student Permit.
Additional documents if applicable:
- Marriage Contract — if married
- Birth Certificates of children — for listing as beneficiaries
- Always present originals when submitting photocopies for verification
How to Submit the SSS E1 Form
Option 1 — Apply Online (Fastest — No Printing Required)
- Go to sss.gov.ph and click “No SS Number yet? Apply Online”
- Complete the electronic form — make sure your name and date of birth exactly match your PSA Birth Certificate
- Submit the form online
- A link will be sent to your email — click it to download and print your Personal Record Form and SS Number Slip
Option 2 — Submit in Person at SSS Branch
- Download and print the E1 form (two copies)
- Fill out the form completely in black ink, capital letters
- Bring your completed form and required documents to the nearest SSS branch
- Present originals of all documents for verification
- Your SS Number will be issued on the spot or within a few days
Option 3 — Through Your Employer
If you are a new employee, your employer is legally required to assist you in registering with SSS. Ask your HR department — many employers process the E1 form on behalf of new hires.
How to Get a Copy of Your Existing SSS E1 Form
If you are already an SSS member and need a copy of your E1 form or personal record:
- Log in to your My.SSS account
- Go to Member Info → Member Details
- View and print your digitized personal profile — this holds the same legal weight as the original E1 form
If your records were not digitized or you have account issues, visit any SSS branch with a valid ID to request your records in person.
Important Reminders
- One SS Number for life — your SS number is permanent. Never apply for a second number — this is a violation that can freeze your contributions and benefits
- Fill out in two copies — one for SSS, one for your personal records
- Capital letters, black ink only — forms with incorrect formatting may be rejected
- Always write N/A — never leave sections blank, write “N/A” if not applicable
- Employers are legally required to collect your SS Number when you are hired under RA 11199
